The Bloom Integrative Wellness Program is a new level of care at Bloom Family Health & Wellness—designed for patients seeking a deeper understanding of their health through a personalized, whole-person approach. By providing the time to perform comprehensive root-cause analysis, review advanced laboratory findings, and understand the many factors influencing your health; together we create individualized, goal-oriented wellness plans focused on whole health optimization, disease prevention, and helping you achieve lasting wellness and vitality.
What Is Functional Medicine?
Functional medicine is a personalized, science-based approach to healthcare that focuses on identifying and addressing the underlying causes of illness rather than simply managing symptoms. Using the functional medicine framework developed by the Institute for Functional Medicine (IFM), we systematically look at each unique factor that influences your health, including your genetics, environment, lifestyle, nutrition, stress, and life experiences.
We don’t just treat a diagnosis; we treat the whole person, recognizing that every individual has unique physical, mental, and emotional needs. By understanding how these factors interact, we develop personalized care plans that help restore balance, optimize function, and support lasting health and well-being.

This program is completely optional. We will continue to accept insurance and provide the same high-quality primary care you’ve always received within the services and visit structure covered by your insurance plan. The Bloom Integrative Wellness Program offers additional time and services beyond what is typically covered by insurance, allowing for a more comprehensive, personalized approach to your health. Whether you choose traditional primary care or our Integrative Wellness Program, we’re committed to providing exceptional care and helping you achieve your health goals.
What’s Included in Your Program Membership
90-Minute Annual Integrative Wellness Evaluation
Your annual visit includes a thorough review of your health history and timeline, an in-depth lifestyle assessment covering nutrition, movement, sleep, stress, and environmental factors, advanced lab review and interpretation, hormonal evaluation, and the development of your personalized wellness and prevention plan.
55-Minute Extended Follow-Up Visits
Follow-up visits focus on reviewing your progress, tracking lab trends over time, refining your nutrition and lifestyle plan, adjusting supplements as needed, and updating your wellness goals. Visits are scheduled based on your needs and the pace of your plan.
Advanced Lab Review and Interpretation
Lab review goes beyond standard normal/abnormal readings. We assess patterns across multiple systems—cardiometabolic markers, hormones, nutrients, and inflammation—to evaluate how your body is functioning and identify areas for optimization.
In-Office Lab Draws
Lab draws are available in-office for participating patients. For labs outside standard insurance coverage, a self-pay option is available and reviewed with you in advance.
Personalized Wellness and Prevention Planning
Your wellness plan is built around your health history, goals, and lab findings, and updated over time. It includes both short- and long-term strategies for prevention, risk reduction, and health optimization.
Nutrition, Lifestyle, and Supplement Guidance
Recommendations cover nutrition patterns, meal timing, movement, sleep, stress, and environmental factors. Supplement guidance—including dosing, timing, and rationale—is provided when appropriate. Program members also receive a 10% discount on supplements.
Priority Scheduling
Program members receive priority access when scheduling visits.
Secure Portal Messaging
Communicate with your care team between visits through the secure patient portal.
Waived On-Call Fee
The standard on-call fee is waived for program members. On-call services remain available per standard guidelines.

During this visit, we will:

Will you still accept my insurance?
Yes, absolutely! We will continue to accept insurance and provide traditional primary care services as we always have. The Bloom Integrative Wellness Program is an optional, self-pay addition that includes services not typically covered by insurance.
Do I need to join this program to continue my care?
No. Participation is completely optional. You may continue with traditional primary care as always, and we will continue to support you the same way we always have.
Is this concierge medicine?
No. This is not a concierge or access-based model.
Why isn't this covered by my insurance?
Insurance coverage and reimbursement guidelines have evolved over time, and many of the more comprehensive, time-intensive services included in this program are not consistently supported within standard visit structures.
Can I still see my provider?
Yes. You can absolutely continue care with your provider. As schedules evolve to include more integrative care visits, if availability does not meet your schedule, we will work with you over time to ensure a smooth transition based on your preferences.
What is included in the program?
The program includes a 90-minute annual Integrative Wellness Evaluation, 55-minute follow-up visits, advanced lab review, in-office lab draws, personalized wellness planning, nutrition and lifestyle guidance, priority scheduling, secure portal messaging, and waived on-call fees.
What if I only want certain parts of this?
That's completely okay! We'll work with you to determine the approach that best supports your health goals and discuss the options available to you.
Will I still use my insurance for labs and visits?
Yes. Covered medical visits and standard laboratory services will continue to be billed through your insurance when appropriate. We'll discuss any non-covered services before they're ordered.
What is an ABN?
An Advance Beneficiary Notice (ABN) is a form that explains when a service or lab may not be covered by your insurance so you can decide whether you'd like to proceed.
What does the program cost?
